This patient underwent a subglandular, saline breast augmentation in 2004. As you can see in the pre-operative photos, during the intervening 6 years she has lost significant upper pole fullness and had noticeable rippling in this upper pole when bent over (this is why I do not perform subglandular breast augmentations). Also, the saline breast implants were overfilled and too firm for the patient. I removed the subglandular, moderate profile saline breast implants, removed the capsules (necessary to allow the space to close) and placed subpectoral, low profile silicone gel implants. The result is a much more natural appearing and feeling result. I suggested a periareolar breast lift as well to make the breasts more symmetric with more elevated nipples. However, the patient elected not to have this additional procedure performed. Still, a much improved appearance.
